Contact IT immediately

If your OTP card is lost, stolen, damaged, or not working for any reason, contact the IT Help Desk immediately. Do not wait.

Submit an MFA Service Request or call the IT Help Desk directly.

If your card may have been stolen or compromised

Report it to the IT Help Desk immediately — even if you are not certain. A compromised card combined with your password could allow unauthorized access to your account. IT can disable the card right away to protect your account while a replacement is arranged.
