Learn how to create a quick survey or form in Microsoft Forms, including the settings to check before you share it and where to find your responses.
What Forms is for
Microsoft Forms is available to all faculty and staff for quick surveys, event feedback, sign-ups, and one-time data collection. You build it yourself, use it, and let it go when you're done.
Microsoft Forms are disposable
A form you create belongs to your account. When you leave NCU, the form, its responses, and any uploaded files leave with you.
If losing the form would cause a problem, don't build it in Forms. See Creating Forms to request FormAssembly or TDX instead.

Before you share
Check these settings first. Open the … menu and choose Settings, or use Collect responses to choose who can respond.
Settings to check
- Who can respond. Choose anyone with the link, or only people at NCU. File upload questions only work when the form is limited to people at NCU.
- Record names. Collects each respondent's name. Only available when the form is limited to people at NCU.
- One response per person. Stops duplicate submissions. Also NCU-only.
- Start and end date. Closes the form automatically, so it doesn't keep collecting after you're done.
Share and see results
Sharing and responses
- Share it. Select Collect responses to copy a link, get a QR code, or embed the form on a web page.
- See results. The Responses tab shows a live summary, charts, and individual responses.
- Work with the data. Select Open in Excel to get a workbook of responses that keeps updating as new ones come in.
Need more than Forms can do?
Payments, approvals, custom branding, advanced logic, or file uploads from people outside NCU all need a different tool. See Creating Forms for what each tool does and how to request one.
