Creating Forms

Learn which form tool to use at NCU, including Microsoft Forms for quick self-service forms, TeamDynamix for requests and approvals, and FormAssembly for permanent and customer-facing forms.

Choosing a form tool

Pick a tool based on how long the form needs to last and what it has to do.

Microsoft Forms is self-service and meant to be thrown away when you're done. For anything permanent, public-facing, tied to a request process, or taking payments, OIT builds it in TDX or FormAssembly.

Microsoft Forms

Self-service

Microsoft Forms is available to all faculty and staff at forms.office.com. Build it yourself, use it, and let it go when you're done.

Best for: quick surveys, event feedback, simple sign-ups, and one-time data collection.

Microsoft Forms are disposable

A form you create belongs to your account. When you leave NCU, the form, its responses, and any uploaded files leave with you.

If losing the form would cause a problem, don't build it in Forms. Request FormAssembly or TDX instead.

When Forms isn't enough

  • File uploads are NCU-only. Upload questions only work when the form is limited to people at NCU. Anyone outside NCU can't upload files.
  • Branding is basic. A few themes and a background image, with no control over fonts or layout.
  • Logic is simple. Branching can skip to a question based on one answer. It can't combine conditions or insert earlier answers into later questions.
  • No payments, approvals, or routing.
  • Very large forms lose features. Past 50,000 responses, viewing individual responses, charts, and "one response per person" stop working.

TeamDynamix (TDX)

Built by OIT

TDX is NCU's system for service requests and ticket tracking, used by IT, Facilities Management, Housekeeping, Events, Marketing, Academics, the Registrar, and One Stop.

Best for: anything that's part of a request process, needs approvals, or needs tracking from start to finish.

  • Built by OIT. All TDX forms are created by the Office of Innovation & Technology.
  • Routing and approvals. Requests go to the right people automatically, including multi-step approvals.
  • No license needed to approve. Forms can send notifications and approvals to staff who don't use TDX.
  • Tracking and reporting. Requesters see status updates, and departments can report on volume and trends.

FormAssembly

Built by OIT

FormAssembly is for permanent, polished forms, especially ones the public sees.

Best for: customer-facing forms, forms that take payments, and forms that need custom design or advanced logic.

  • Payments. Accept payments directly in the form.
  • Full branding. Custom design, layout, and question types.
  • Advanced logic. Conditions based on multiple answers.
  • Notifications. Flexible email and workflow options.

FormAssembly + Salesforce

Built by OIT

Everything FormAssembly does, connected directly to NCU's Salesforce.

Best for: forms that need to read from or write to Salesforce records.

  • Pre-filled fields. Fill in form fields from existing Salesforce records.
  • Lookups. Search Salesforce data from within the form.
  • Automatic records. Create or update Salesforce records when the form is submitted.

Requesting a form from OIT

Submit a request

TDX forms: submit a TDX Service Request describing the request process and approvals you need.

FormAssembly or FormAssembly + Salesforce: submit a FormAssembly request.

Either way, include the form's purpose, the fields you need, who fills it out, and any integrations or payments.
