To request assistance or report issues with MFA or the Microsoft Authenticator.

If you are locked out of your account/unable to log in and cannot access the form, or lost access to your MFA device, immediately call or visit the IT Service Desk (612.343.4170) for assistance with your account. The IT Service Desk will need to take action on your account to resolve.
