Final Exam Exception

Final examinations must be taken at designated times, except in the case of a life-threatening family emergency with documentation or a critical illness documented by a physician/health care professional. Other extreme instances may be given. If a schedule adjustment is necessary, please complete this form no later than two weeks prior to the first exam scheduled.
